Season 1
- My First Day (aka Pilot) — J.D. and the rest of the interns begin their first day at Sacred Heart.
- My Mentor — J.D. tries to get to know Dr. Cox. Elliot gets on the wrong side of Carla.
- My Best Friend's Mistake — J.D. misses spending time with Turk, who he also thinks has made a mistake during surgery.
- My Old Lady — The three new interns have to confront death for the first time.
- My Two Dads — J.D. has to decide between Dr. Kelso and Dr. Cox. Elliot thinks her breasts have healing powers and Turk gives Carla a gift of a pen that unbeknownst to him was removed from someone's rectum.
- My Bad — J.D. is given a board member to care for, whom with he has sex. She turns out to be Dr. Cox's ex-wife, Jordan.
- My Super Ego — J.D. is brought down a couple of pegs by another intern.
- My Fifteen Minutes — J.D. and Turk save the life of a TV cameraman, while at the same time worrying about intern evaluations.
- My Day Off — J.D. develops appendicitis and sees the hospital from the patients' point of view.
- My Nickname — J.D. and Carla's relationship changes when he starts to have more medical knowledge than her.
- My Own Personal Jesus — Turk loses his Christmas spirit after spending a night on call. He regains it after helping a pregnant girl.
- My Blind Date — J.D. is tasked with looking after a social worker who slipped and fell in the hospital. She is stuck in an MRI machine and he asks her out without seeing her face.
- My Balancing Act — J.D. and the social worker, Alex, are going out. However they are continually interrupted by the hospital. Carla is unable to have an orgasm.
- My Drug Buddy — A patient is suspected of stealing drugs; tt turns out to be Alex herself. J.D. and Elliot sleep together at the end of the episode.
- Bed Banter & Beyond — J.D. and Elliot spend the day in bed having sex, while flashing forward as their relationship deteriorates.
- My Heavy Meddle — J.D. and Elliot are not talking to each other after breaking up. Dr. Cox goes on the rampage.
- My Student — The interns receive their first interns, each of whom is very different. J.D.'s is very much like he was at the beginning; Elliot's is a jerk but the son of the CEO of the corporation that owns the hospital; Turk's is a smart confident woman who Dr. Cox finds he's attracted to. Guest-stars Kelli Williams as Kristen Murphy.
- My Tuscaloosa Heart — J.D. feels guilty when a rude patient, who he ignored, dies. Carla assures him that it was the terminal cancer, but J.D. is unable to sleep. Dr. Cox is unable to decide between the three women who he likes. Elliot and Turk discover a new side to Dr. Kelso.
- My Old Man — The interns' parents come to visit. Guest-stars R. Lee Ermey as the janitor's father, and John Ritter as Sam Dorian.
- My Way or the Highway — J.D. is angry at Turk for convincing his patient to have surgery.
- My Sacrificial Clam — J.D. is stuck by a needle full of blood contaminated with Hepatitis B. He becomes scared of getting sick. Elliot chooses the hospital over her new boyfriend.
- My Occurrence (Part 1) — Jordan's brother comes into the hospital after piercing his hand with a nail gun. His hand won't stop bleeding and the doctors suspect that he has leukemia. J.D. is unable to accept this diagnosis. Guest-stars Brendan Fraser as Ben Sullivan.
- My Hero (Part 2) — Ben begins treatment for his cancer, but Dr. Cox is unable to be there as he loves Ben too much.
- My Last Day — The interns realize they have become the jaded doctors they said they never would. Jordan ends the episode by dropping a bombshell.
Season 2
- My Overkill — In the aftermath of Jordan's revelations, everyone is estranged from each other. After Jordan revealed that she slept with J.D., he expects the worst from Dr. Cox. Elliot, embarrassed by Jordan's revelation about her lingering feelings for J.D., is avoiding J.D. at all costs. And while Carla is angry with Turk for not telling her that Dr. Cox was in love with her, Dr. Cox is annoyed at Dr. Kelso for being played for a non-existant promption, and at J.D. for sleeping with Jordan. Guest-stars Colin Hay as the troubador.
- My Nightingale — The residents become very excited when a televised car chase comes to a dramatic end with the car crashing in the hospital parking lot. Excited to be in the middle of the action, Elliot gets to treat Officer Berson, one of the cops who was admitted with chest pains and Turk treats the driver of the car.
- My Case Study — Dr. Kelso starts an impromptu competition with the residents, offering a all-expenses-paid medcial conference outing in Reno if they can bring him the most interesting case study: J.D. is torn to compete and to keep in Dr. Cox's favour by not doing so. Another competiton begins when the entire hosptal takes advantage of Dr Kelso's annual good mood the next day. Elliot worries that she isn't getting on well with Carla.
- My Big Mouth — J.D. and Carla's relationship deteriorates when J.D. accidently lets out some of Carla's secrets. The surgical residents compete for a trip to Mexico with Dr. Kelso. Elliot worries that she isn't presenting a competent image to Dr. Cox and takes on some harrowing cases to improve it.
- My New Coat — J.D. gets an ego boost by wearing a white doctor's coat, but also has to accept the increased responsibilities that come with it. Elliot's reputation takes a knock when she has a one-off fling with a doctor she'd never previously met.
- My Big Brother — J.D.'s older brother drops by for a visit, but J.D. can't hide how ashamed he is of his less-successful life. Turk learns a lesson when he makes a bet with Dr. Cox if a patient lives or dies. Halloween has a mysterious person in an ape-suit running around the hospital, much to Dr. Kelso's disgust of the holiday. Guest-stars Tom Cavanagh as Dan Dorian.
- My First Step — Dr. Cox and the drug-promotion agent — which everyone has a secret crush on — clash, as do J.D. and Elliot over who's the better doctor. Carla worries over ambitions about "just being a nurse". Guest-stars Heather Locklear as Julie Keaton.
- My Fruit Cups — Dr. Cox and Julie hook up. J.D. and Turk's financial situation is highlighted with an insight on their food and toilet-paper thefts in the hospital; Elliot's is also when her father cuts her off after refusing to become an OBGYN at his request. A newly-pregnant Jordan reappears, and Dr. Cox is faced choosing between her or Julie.
- My Lucky Day — Elliot is forced to move out and also has to deal with a malpractice suit; J.D. shows up Dr. Cox and an unofficial competition begins between them. Guest-starts Alan Ruck as Mr. Bragin, and a cameo appearance by David Copperfield.
- My Monster — Turk and Carla have relationship problems, as do Dr. Cox and Jordan. J.D. considers exactly how much the hospital consumes their lives; Elliot is forced to live in the back of a truck as she has no time to find another place, and J.D. loses his mojo while dating Lisa the gift-shop girl. J.D. later offers Elliot to stay with him and Turk, and they end up sleeping with each other again.
- My Sex Buddy — Not wanting to repeat their last disasterous relationship, J.D. and Elliot agree to become sex buddies to limit their feelings for each other, although this doesn't entirely work out as they planned. Turk tries to help out Elliot with her workload, which backfires; and Carla learns a lesson about why hospitals can't always give definite answers.
- My New Old Friend — J.D. has to deal with keeping his growing feelings for Elliot after they break off having casual sex; he tries seeing Lisa again which raises tension between them. Dr. Cox and Carla misdiagnose a hyperchondriac patient after not taking his claims seriously, and Dr. Kelso teaches Turk a lesson about the small lies their patients tell them. Guest-stars Richard Kind as Mr. Corman.
- My Philosophy — Elliot demands single-sex change rooms from Dr. Kelso as it's becoming increasingly embarrassing changing in the traditional single-sex rooms. Turk pops the question to Carla, but not in the way he'd originally planned.
- My Brother, My Keeper — Turk's brother Kevin comes to visit with news of his own to share. An old doctor friend of Dr. Kelso's who everyone gets on well with (the "anti-Kelso") runs into trouble when J.D. screws up an out-dated procedure under his supervision. Guest-stars Dick van Dyke as Dr. Townshend.
- His Story — Narrated from Dr. Cox's viewpoint: shows Dr Cox and his sessions with his just-as-terse shrink, and with his difficulties with Dr. Kelso and also with J.D. Elliot is both excited and dismayed to find herself attracted to Paul, a male nurse. After delaying for so long, Carla finally says yes to Turk. Guest-stars Eric Bogosian as Dr. Gross, Dr. Cox's psychologist.
- My Karma — J.D. and Turk attempt to cover up the fact they were hitting golf balls off the roof and may have caused the accident their patient has suffered from, only the janitor also knows. Jordan finally gives birth, but slips out in front of J.D. that Dr. Cox is actually the father, and asks him not to tell him.
- My Own Private Practice Guy — J.D. has a patient who has her own private practive doctor much like himself, but later realises he was the reason Jordan and Dr. Cox divorced. The janitor starts flirting with Elliot, and Carla has issues with her appearance. Guest-staring Jay Mohr as Dr. Peter Fisher, and a cameo appearance by Jay Leno.
- My T.C.W. — Dr. Cox switches from female to dog names for J.D. after an interesting bestiality rumour does the rounds. Dr. Cox himself is barely getting any attention from Jordan over the baby, Elliot and Paul — and Turk and Carla — are fighting about their relationships, and when J.D. accepts a date from Jamie, the very attractive wife of a coma patient, and comes under fire for it, he eventually snaps.
- My Kingdom — When J.D. does a temporary surgery elective, his friendship with Turk suffers to win popularity. The janitor gets a new powersaw, and Dr. Cox's prank on Dr. Kelso while he's away gets a little out of hand. Elliot's slip of the tongue drastically changes her relationship with Paul.
- My Interpretation — J.D. attends the funeral of the TCW's husband, with predictable results. Turk has a sex-dream about Elliot, but Carla finds out about it. Dr. Cox's impression of Jordan's baby continues to slide rapidly downhill.
- My Drama Queen — J.D.'s relationship with the TCW takes a few interesting twists when Elliot correctly identifies her as a drama queen. Carla's mother dies. Dr. Cox is stuck with Ted in teaching polite doctor-patient relations after having Dr. Kelso witness a particularly acidic tongue-lashing of his at a patient.
- My Dream Job — The end of the second year has the residents somewhat bored with the monotony of their jobs, until an old college buddy of J.D. and Turk's turns up, and accidently reveals to Dr. Cox that Jordan's baby is also his; further antics have them both dismissed from their on-call shifts when they arrive after drinking. As Dr. Kelso attempts to break Elliott's spirit, Dr. Cox tries to cope with the shock that he's a father, but Jordan can't lose him again after just getting him back. Cox eventually confesses to J.D. that he isn't angry at Jordan for lying to him; he's scared that he'll be a bad father. J.D. reassures Cox, pointing out how everyone at the hospital looks up to him, and the way he disciplined him and Turk for drinking on a work night. This prompts Cox to intervene when he sees Kelso publicly humiliating Elliott. Guest-stars Ryan Reynolds as Spence.
Season 3
- My American Girl — J.D, Turk, and Carla work together to find out what is wrong with one of J.D.'s patients. Elliot, frustrated that everyone walks all over her, and desperately needing to change her bad-luck streak, acts on some advice from the janitor and gets a make-over.
- My Journey — Turk and Carla set the date for their wedding; J.D. is worried about losing how close he always was with Turk and asks him out on a "man date". Elliot gets back with her ex-boyfriend Sean, but the relationship has a rocky start. Carla is obsessed over finding whose urine is in an unmarked container she comes across.
- My White Whale — Jordan and Dr. Cox worry about a slight cough their son has and try to get him seen by the hospital's own paediatrician, who is just as arrogant as Dr. Cox is. The residents get their own interns: Turk is too firm, J.D. isn't firm enough, and Elliot is a mix of both, especially when Dr. Kelso compares the clumsiness of one of them to her own. Guest-stars Christopher Meloni as Dr. Dave Norris.
- My Lucky Night — Elliot admits she a problem saying 'dirty words' for the human body after accidently giving a patient an orgasm during a pelvic examination. Carla realises she may be too assertive for her own good. J.D bears the consequences of giving advice to Dr. Cox about his relationship with Jordan and his son.
- My Brother, Where Art Thou? - Carla and Elliott moonlight at a vet's. J.D.'s big brother turns up and is dismayed to find out that J.D. has become a cynical, jaded mini-Dr.-Cox.
- My Advice to You
- My Fifteen Seconds — Carla regrets advising Elliott on how to treat a patient; Kelso temporarily goes deaf; J.D. and Cox can't work out how a patient managed to inhale pesticides.
- My Friend the Doctor — Elliott worries that she still doesn't really consider herself a doctor; J.D. discovers a shocking secret about the janitor's past.

- My Screw Up — Jordan's brother (and close friend of Dr. Cox) Ben dies. Dr. Cox is in denial and blames J.D. for the death.
- My Tormented Mentor — A new female attenting surgeon starts. Dr. Cox is having a hard time overcoming Ben's death. Carla teaches Dr. Cox, Kelso, and Todd in a class about sexual harassment.

- His Story II — During surgery Turk causes nerve damage to a young pianist who he can't admit his mistake to. J.D. and Elliot sleep together, then right afterwards Sean arrives and Elliot goes off with him.
- My Choosiest Choice of All
- My Fault — J.D. finally breaks up with Danni. Dr. Cox battles Kelso over the hospital offering full body scans to healthy patients. Elliot dumps Sean and gets with J.D. — who immediately realises he doesn't want her.
- My Self-Examination — Carla's brother comes to stay for the wedding. At the rehersal dinner, J.D. tells Elliot that he doesn't love her.
- My Best Friend's Wedding — The day of Turk and Carla's wedding. Turk is delayed at work, and when he finally finishes he goes to the wrong church. The ceremony is cancelled. J.D. goes to see Sean and trys to convice him to get back with Elliot. Turk and Carla finally get married when they return to hospital to check on a patient who turns out to be a priest.
Season 4
- My Old Friend's New Friend — Molly Clock (Heather Graham), the hospital's new shrink, becomes friends with Elliot whilst Turk, JD and Carla make the impression of shutting her out
- My Office — JD and Elliot are named the hospital's new chief residents by Dr. Cox
- My New Game — JD is annoyed that the janitor keeps spreading the word that he is the Co-Chief Resident and Elliot the Chief Resident
- My First Kill — JD thinks about whether he has ever specifically let someone die through his own incompetence and comes to the conclusion that he has not yet, though Dr. Cox is of the impression that it will happen any day now.
- Her Story — Told from the perspective of Elliot, she starts to doubt Molly being the perfect mentor when she finds out her boyfriend is a convicted felon. Nevertheless, Dr. Cox helps her decide that personal lives should be kept seperate from professional lives. Molly breaks up with her boyfriend anyway.
- My Cake — JD's dad dies, and his brother Dan comes to visit. Cox and Dan promise each other to help JD get out of his depression.
- My Common Enemy — Molly becomes the enemy of Cox and Kelso by wanting to prove to them that everyone has a "creamy centre" and no one is evil.
- My Last Chance — Molly and JD are on the brink of sleeping together, but must first get the permission of Elliot.

- My Hypocritical Oath - J.D. becomes attracted to a girl, then finds out her new boyfriend has an STD. However, he can't tell her because of patient/doctor confidentiality.
- My Quarantine - When J.D. casually remarks that one patient's symptoms resemble SARS the entire staff is trapped in quaranine for the night. J.D. pays a homeless guy to fake a heart-attack so he looks more "doctorly" in front of his date.
- My Life In Four Cameras - When Cox fumes at Kelso for firing another staff member due to budget cuts, Kelso challenges Cox to balance the budget without firing anyone. Meanwhile, a sitcom writer is admitted. J.D. starts to fantasise about what it would be like if the hospital was a sitcom- complete with laugh track.
- My Roommates — Turk and Karla ask JD to move out to give them space. However, his absence causes them to realize they have a problem with their relationship. Meanwhile, Dr. Cox diagnoses his friend's child with Autism.
- My Best Laid Plans - Molly returns to Sacred Heart, and J.D. blows off his girlfriend to go out with her. Carla tries not to make "mountains out of molehills" when she realises Turk is still in contact with an ex-girlfriend. The janitor bets Dr. Cox that he can get a date with Elliott.
- My Boss's Free Haircut - Carla is staying at Elliott's while her marriage to Turk hits the rocks. JD takes Turk out to cheer him up; Elliott tries and fails to cheer Carla up the same way. Kelso warns Cox about the number of complaints patients have made about his blunt remarks.
