West Ruislip is the western terminus of the Central Line, and a stop on the Chiltern Railways service from Marylebone towards High Wycombe and Birmingham Snow Hill.

The station's platforms are aligned in an island layout, with the lines extending past the platforms under the road bridge westwards. This is a legacy of the original design proposals, which saw Central Line services extending to Denham. This extension was abandoned following the post-war passing of the Town and Country Planning acts, which blocked the continuation of the pre-war speculative housing development which the extension would have served.

The depot to the east of the station handles many stock movements into and out of the station as trains come into and out of service.

Two more platforms for the Chiltern services are located north of the Central Line platforms. The original layout had two through roads and two platform roads, but simplification of the layout when the old manual signal box closed removed the westbound platform road and widened the westbound platform. Only the eastbound services into Marylebone now have a through and a platform road. Chiltern Railways have indicated that four roads may be restored at some time in the future.

A shunting neck is located west over the road overbridge on the south side of the line, to allow stock and freight (mainly ballast train) movements into and out of West Ruislip LUL depot. On the north side of the line are two sidings, at present used only for the occasional ballast train. For a period some years ago, a shuttle service to Marylebone used the sidings as a reversing point; this service has ceased, and all trains now run through.
