The original version of the article mentions that the text is "copied with permission", but does not give any further details; a later version by the same user has expanded it with further material from the page. There has been significant further material since then, but major structural portions of the article are from this source, and anything not from there is uncited for other sources. Copyright attribution on the source is "Recovery Specialties, LLC. All rights reserved. 2007", so it is not of the type that would permit wholesale inclusion.
